TOUCH Cluster Support (TCLS) aims to enable vulnerable elderly, who have little or no family support, to age-in-place in the neighbourhoods of Kallang / Novena and Yishun / Sembawang.

 

Social workers will assess, support and monitor seniors who needs help in financial assistance, housing and case management.

Social workers also coordinate help for these seniors, such as in activities of daily living, home medical and nursing, home therapy, personal hygiene, providing meals and transport for medical appointments.

Through its work, TCLS hopes to see:

  • Vulnerable seniors supported to age-in-place with community support
  • Seniors actively engaged in the community
  • A local social and care support network established for seniors through effective partnership and networking with other agencies in the community
